Requesting an Applicant Agreement by Mail
Applicants who are unable to download and print the Applicant Agreement from this web site may request that an Applicant Agreement be sent to them by regular mail, as described below.
Please Note:
- If you are able to download and print the Applicant Agreement from this web site, you should not also request that an Agreement be sent to you by regular mail.
- Submitting a request for an Applicant Agreement from this web site does NOT, in itself, register you for the Match. An Applicant Agreement will be sent to you by first class mail. Please note that it may take up to 2 weeks for you to receive this package.
- To register and obtain an applicant Code Number for participation in the Match, you must complete and sign the Applicant Agreement and return it to National Matching Services Inc. (NMS), together with the non-refundable applicant registration fee.
- Registering for the Match does NOT constitute an application to any of the programs participating in the Match. You must still apply directly to programs in which you are interested. For information on applying directly to residency programs click here.
Requesting an Applicant Agreement:
- Within the United States (including Puerto Rico) and Canada: CLICK HERE to request that NMS send an Applicant Agreement to an address located in the United States or Canada.
- Outside of the United States and Canada: CLICK HERE to request that NMS send an Applicant Agreement to an address located outside of the United States and Canada.
