Requesting an Applicant Agreement by Mail
Students who are unable to download and print the Applicant Agreement from this web site may request that an Applicant Agreement be sent to them by regular mail, as described below.
Please Note:
- If you are able to download and print the Applicant Agreement from this web site, you should not also request that an Agreement be sent to you by regular mail.
- Submitting a request for an Applicant Agreement from this web site does NOT, in itself, register you for the Match. An Applicant Agreement will be sent to you by first class mail. Please note that it may take up to 2 weeks for you to receive this package.
- To register for the Match, you must complete and sign the Applicant Agreement and return it to National Matching Services Inc. (NMS), together with the non-refundable $60 registration fee.
- Registering for the Match does NOT constitute an application to any of the programs participating in the Match. You must still apply directly to programs in which you are interested. Applications to programs are submitted using ERAS, which is a separate and distinct service from the Match.
Complete and submit the form below to request that NMS send you an Applicant Agreement. If you have more than one mailing address you must specify only one address where you want the Agreement sent. You may not request that NMS send multiple mailings to different addresses.
